Supporting people to develop Person Centred Plans
Date: Thursday 30th January
Time: 10am – 12.30pm
What does a good Personal Outcomes Plan look like? This session will explore how to identify and write Personal Outcomes and how to relate these to the support people are assessed as needing.
We will explore how to build-in ‘choice, control and flexibility’ to Plans and the different formats that they can take. Plans for children, young people and unpaid carers will also be considered.
In addition, we will look at how Personal Outcome Plans can be used to support the assessment and review process, helping people to identify what support they need in the future.
There will be an opportunity to share ideas, consider different examples and talk through the process of identifying Personal Outcomes and supporting people through the process.
The sessions will be delivered by Your Options Understood and VOCAL.
This session will take place in January 2025.
SDS Scotland members will be able to access discounted tickets, with non-members also being able to purchase tickets for this event. A small number of free places have also been made available to people using SDS, unpaid carers and Local Authority staff.
